Question for Dr Q ?

I have PCOS and was tested via blood last week to see if I was insulin resistant...as I'm TRYING to CONVINCE my

OB that I want to take Metformin, I've read and heard personal stories about PCOS and Met.......

I am insulin resistant.

HOWEVER...my OB prescribed me Glyburide instead.....what gives? Will it do the same thing with PCOS or should I call them and re-ask for Metformin?

Thank you!

~Dee

Could it be?

I had a tubal reversal in April, 07 and we still have not gotten pregnant. I have only one tube and one ovary. We were resigned to the fact it was lack of tube function and are praying about doing a round of IVF. I had a check up with our new family doc and he said "I think I know why you aren't getting pregnant: insulin resistance". So I am going back next week to talk about starting glucophage and diet and exercise.

He wants us to try this for 3-4 months before we make an IVF decision...So we are praying this is God opening a door.

Please pray for us! I am excited for the first time in months about ttc!

PCOS Diagnosis

I was tested for PCOS when I was 17, but the doctor ruled it out because my insulin and testosterone levels were normail. I did have several small ovarian cysts, but nothing abnormal she said. I am now 24, and have recently been tested and diagnosed with PCOS even though my testosterone and insulin levels are normal. I have never had a period without the help of birthconrol. Could there be something else wrong with me?

Docs: Can clomid cause insulin resistance?

For years, my pituitary endo has suggested I might have a slight insulin resistance. (Incidentally, I see an endo for high prolactin-under control with cabergoline--2 pills 2x/weeke) And she said to be especially aware of how my body is reacting to sugar since I had gestational diabetes with my daughter. There has been some talk of putting me on metformin for the IR.

Now, we're trying for #2. Last month was my first month on clomid. I found myself hungry all the time. I ended up putting on 4% of my bodyweight in a month.... quite discouraging since it took me 6 months to loose 10% and now 4% is back.

I'm wondering if Clomid, since its job is to boost some hormone levels, could it have caused an insulin resistance to show up? I hate to put on all the weight I worked so hard to loose last year. Is it now time for me to bite the bullet on metformin?

BTW: My testosterone levels were normal at last check, so my endo didn't think I had PCOS. My cycles are relatively regular on the cabergoline.

What do you think?

PCOS without cysts or insulin resistance?

Hi All,

I have a question about the use of Metformin to treat suspected PCOS *without* insulin resistance.

I have twice been told that I probably have PCOS (once about 5 years ago and again just recently), but both times when tested I have not been insulin resistant or had elevated androgens or had any cysts on my ovaries.

I am 33 years old, not obese (5'8 and 155 lbs), but I have always had extremely irregular cycles (in the last year from 29 days to 75 days) and since a miscarriage last year I have been unable to conceive and have had anywhere from a few days to a few weeks of spotting or light bleeding in the middle of every cycle.

I also have a bit of adult acne and heavier/darker hair growth than many women, but I am of Mediterranean descent and I am not significantly hairier than other women of the same ethnicity.

So my current doc has prescribed Metformin and Clomid. The Clomid I understand, but why Metformin if I am not insulin resistant and have only one definite symptom (irregular cycles) of PCOS?

Pretty much everything else has been ruled out - thyroid is normal and I don't have any polyps or fibroids - so I get the feeling my doc is just trying to treat *something* because she doesn't really know what is going on.

Is this really a good idea or are there potential risks to taking Metformin if I am not insulin resistant?

Thanks in advance for any guidance you can offer!

Dr Q?

When you first go to the RE and they schedule you for a fertility workup, do they automatically test for insulin resistance or is that something you have to ask about? Do you always have to have PCOS if have insulin resistance?

I have high blood pressure, prediabetes, overweight, something is up with my liver enzymes (I don't understand much about this), and my good cholesterol is low. I do not have high bad cholesterol though. Do you think it sounds like insulin resistance to you?

I'm seeing an RE and will be speaking to him about this, but I wanted your opinion too!

Today's Blood Work

Hi Dr. Q,

Today I went to the lab to get my blood work done that was ordered by the RE. They only did one draw for everything even glucose and insulin. If they were testing for IR shouldn't I have drank that sugar stuff? Is it possible that the RE can use my fasting insulin and glucose to determine if I am IR or is this an inaccurate way of doing such? Thanks!

Trying to Conceive

I am 32 years old and have never had a regular period. My fiance and I are trying to conceive, but when I tried with my ex-husband I was told I don't ovulate properly. My current ob/gyn has me on a Provera regimen, 10mg for 10 days each month to try to kick start a cycle, but I took the provera from 7/23 to 8/1 and so far all I have had is a little spotting and some extreme cramping. I have read up on Vitex and question whether it is safe to use with the provera. Any suggestions?

PCOS no period

Hi everyone. I have been taking metformin for 7 months b/c of a diagnosed insulin resistance issue. I am thin (5'5" and 110 lbs.) so don't fit the "normal" diagnosis for PCOS. Basically, since I've been on the metformin, I've stopped having period entirely. I used to bleed every 35-40 day or so, nothing "normal," but at least a period! Now, I can go 90 days without, and the only way I do have a period is with estrogen and provera. Anyone out there with similar issues? I'm not sure what to do at this point... I'd appreciate any feedback - thanks in advance!!
