Clomid

I had clomid-IUI cycle in April. I did not conceive in that cycle. Next month I got pregnant natrually, without any treatment. But I miscarried in 5th week. Doctor was not sure whether it was tubal or not, since no sack was visible. They gave me 2 doeses of MTX to end the pregnancy as isnpite of heavy bleeding, HCG was not getting zero on its own.

After that I had 2 regular/normal periods. But last 2 periods have been with 'very less' flow. The latest one, only lasted 2 days, with less flow. Though I had normal symptoms of ovulation like bloating and sore breasts from mid-cycle. I have never experienced such less flow before. Any idea why this would have happened?

Also, I am trying another Clomid cycle this month, as suggested by my doctor. I am 30 yrs old.

My AF showed up today at 4pm. Do I count today as CD 1 or tomorrow? It is full flow.

I've read that some docs give a time cut off.

Doctor(s)?

Others? What do your docs have you do?

You are not welcome in my home

You are not welcome in my home. I am officially getting a restraining on you today before you just "show up". You never call or write..just show up. What is up with that? Everytime you come..you ALWAYS make a mess. You cause intentional pain and suffering. I have always call you "aunt" but have never considered you family. I am disowning you. I got rid of you for good three years ago, but you will not be visiting my surrogate for the next 40 weeks or longer. Good bye Mrs. Flow...get out of our lives.

Dr Q... Ladies... Spotting question after laporscopy, hysteroscopy, and d&c

Hello. I have a question. I had a laporoscopy, hysteroscopy, d&c (d&c not due to a m/c) and dye study 8 days ago (last Tuesday). My regular menstrual cycle is due Aug 1. They told me I may start earlier or later. I need to contact my dr office when I start because i need to have another dye study between cycle day 5-10 (both tubes were blocked at time of surgery, blockages were removed, and am having another dye study to see if it was successful). I spotted the first day after surgery then nothing since. Yesterday I started having af cramps and started spotting brown blood. Today I still have cramps but the spotting has turned to bright red but is VERY light like spotting. I don't know if this is AF but just really light and early or not. I need to know because I need to have the dye study scheduled. WHat do you think? Thanks!

Light period

My LMP was may 26 but I didn't get a heavier flow until day 4. It didn't last very long...light bleeding on days 1-3, slightly heavier flow on day 4, then light and tapering off by day 5. This is unusual for me. If AF doesn't come by the 20th of June, I'll have to test.

is there a problem?!?

hello again..

earlier i posted up that i was spotting and had very light period (thought that could be a breakthrough bleed) for a while and now i have my flow. but it seems so weird because my af is really really heavy. usually when i do have my af, it's red for like 3-4 days but only cd 1-3 is heavy then it gets lighter.and my af last for 7 days. but since i had my flow, it's so heavy.

may be there something wrong?

Dr Q..

hi dr. Q..

in january i had a breakthrough bleed.. so the ending of the month i had my real af.. then on march 2 i began spotting and now it led to more.. but no red blood yet. still dark brown. like what you would have cd5.

could this be considered it as breakthrough bleeding again? (btw..i never used to have any breakthrough, i just have my af once a month)

and my real full flow hasn't begun so could i consider it as my af?

Advanced IVF Technologies

This article will pertain to certain technologies that are available to those who are undergoing In Vitro Fertilization (IVF)

Intracytoplasmic Sperm Injection (ICSI) is a procedure in which a single sperm is isolated and directly injected into the egg, creating fertilization. This technique was revolution when finally mastered in the late 1990s. Prior to this, men who produced few sperm or sperm with low motility (poor swimmers), required donor sperm since their sperm may not have had the capacity of burrowing through the egg’s shell appropriately. Now, although very difficult to catch a single swimming sperm with a pipette, a man who produces a very low sperm count, through ICSI, fertilization can occur.

Testicular Sperm Extraction (TESE)- Usually performed by a Urologist who specializes in Male Fertility. Approximately 40% of men who produce no sperm have some sort of tubal obstruction (yep, sperm have to pass through a series of pipes to be ejaculated) usually in the Vas Deference. Men who have had hernia repairs are usually at high risk for having obstructions created from surgery. Some men, especially Cystic Fibrosis carriers are born with the Vas Deference. In these cases, there is usually ample sperm in the testis and hopefully epididymis where sperm are usually more mature. A Urologist can simply extract sperm directly from the testis with a special syringe, and the eggs after they’ve been extracted from the body through IVF, can be fertilized through ICSI resulting in embryos that are grown in an incubator for 3-5 days and placed back into the uterus.

Assisted Hatching (AH)- Embryos sometimes have shells that surround them that look very thick. We believe that these thickened shells may not allow the embryo to “hatch” sometimes, which wouldn’t allow for implantation and pregnancy. These thickened shells are notoriously encountered in Poor Responders and in women over the age of 38. The shell can be thinned out to make it easier for the embryo to hatch either through a laser or an acid solution.

Preimplantation Genetic Diagnosis (PGD) A procedure where a specific genetic defect can be determined in embryos. A Day 3 embryo usually has 8 cells and one of these cells is extracted and analyzed for the specific defect. For example, a couple both are carriers for Cystic Fibrosis (CF), meaning they are at high risk for passing fulminant CF to their children. The cell that was extracted from the embryo is analyzed for CF, and if the cell doesn’t have this genetic defect, it is assumed the embryo does not and vice versa. Only those embryos that had the normal cells would then be transferred back into the uterus to allow implantation and pregnancy to occur with a non-afflicted baby. Nowadays, sometimes when a family has a child that has a certain type of leukemia and there is no one in the family with a bone marrow match, couples undergo IVF + PGD to specifically look for an embryo that will carry the bone marrow matching gene and only these embryos are transferred back. Once the baby is born, a small sample of the baby’s bone marrow is taken to save his/her siblings life from leukemia.

Preimplantation Genetic Screening (PGS) A procedure in which couples simply want to make sure the embryos being transferred back are chromosomally normal. The biopsy can either be performed on a Day 3 embryo or on a Day 5 blastocysts (embryo). The advantage of doing a day 5 trophoectoderm biopsy is that multiple cells (not just one) can be biopsied from the area of the blastocysts that is destined to be the placenta. Presently, there are two ways to screen embryos, either through Flourescent In-Situ Hybridization (FISH) or Comparative Genetic Hybridization (CGH). FISH can not check all of the chromosomes, so only a few are checked. CGH can check all 46 (including sex) chromosomes and is becoming commercially available. Both of these procedures only check for the number of chromosomes, which can be great to avoid genetic defects that are associated with these, such as Down’s Syndrome. As several articles are pointing out however, the biopsied cells that are being screened with these two technologies are not necessarily representative of the cell itself. Some “abnormal” cells seem to “self-correct” early on and possibly become normal babies and vice versa. Although CGH is more complete than FISH for checking chromosomes, CGH is in its infancy and requires at present the freezing of the biopsied blastocyst with eventual thawing of the “normal” blastocyst for transfer. At present, PGS remains controversial in the fertility world whether screening provides any benefit since we can’t be sure of the chromosomal findings.

Microarray Chips are presently under scientific investigation in which several hundreds of genes (not just the chromosome number) can be evaluated in each embryo. Lethal and/or debilitating genes found in embryos can primarily be evaluated and only those that do not carry these genes can be transferred, but this technology is still a few years away from being commercially available.

There are still many more technologies being developed, such as finding embryo markers in culture media that by simply checking the media and not manipulating the embryo, we may be able to determine the best embryo. Others are studying flow dynamics to try and mimic the fallopian tube environment for the embryo. Whereas these make sense, we are hopeful that within a few years, we will be able to provide these technologies to you to not only improve pregnancy rates, but to also provide diagnosis when needed.
