      <title>Elevated FSH Level</title>
      <description>&lt;div&gt;CD 3 BW just revealed an FSH level of 20 and a low to normal estrogen level. I'm 32 and seeing an RE. Past 3 cycles revealed elevated estrogen levels and normal FSH levels. No cysts founf though, including today's utrasound. Clomid has not worked and this cycle's injectables and IUI Have been cancelled based on my BW results. What could this mean?&lt;/div&gt;

      <title>IVF FAilure- Got AF, now waiting for second one??</title>
      <description>&lt;div&gt;Ok so I am looking for help- I know I have been MIA- please understand that we are going thru alot- but I do need help...here goes:&lt;br/&gt;&lt;br/&gt;When my IVF failed, I got AF on Monday September 22nd (exactly 28 days from my last AF in Aug- even though I had just gone thru a medicated cycle) &lt;br/&gt;&lt;br/&gt;We did not use OPK this month or anything and ony had sex 2 times.. on CD 11 and CD 14 &lt;br/&gt;&lt;br/&gt;I felt "O" pains on CD 13 but we honestly were not trying at all (trying to take pressure off and deal with the failure)... I assumed I would get AF Sunday 10/19/08 (cd 28) But of course I am not 100% sure when I ovulated- I started to feel crampy on Sat, Sunday and a little Monday- as if AF is on its way....but nothing. I am always regular 28 day cycle since age 14 (now 32) &lt;br/&gt;&lt;br/&gt;Do you know if its typical for cycles to be messed up after IVF- or should they resume there normal course of pattern. Even when I was on clomid (3 rounds for IUI- I did not have issues with delayed period and no history of cysts)... And since I did not have to wait for a first period- It actually came 2 days before the beta (9/22) Here we are CD 30 and waiting on a second AF .... &lt;br/&gt;&lt;br/&gt;I should add I did NOT use PIO (progesterone in oil) injections- I did take vaginal progesterone suppositories and stopped them on 9/22/08 &lt;br/&gt;&lt;br/&gt;I am afraid to take a HPT- seeing a negative will kill me- as I am still dealing with my emotions from the IVF failure- &lt;br/&gt;&lt;br/&gt;Please Advise....&lt;br/&gt;&lt;br/&gt;&lt;/div&gt;

      <title>Primary Ovarian failure? Help please</title>
      <description>&lt;div&gt;Doc,&lt;br/&gt;&lt;br/&gt;I am currently on 300iu of gonal-f going up to 375iu. I started with 75iu and then to 150 then 225 and all i got was a 51 on estrogen then 81 and now 90 after 13 days of injections. Does this sound like I could have Primary ovarian failure if 300iu only took me to a 90? I have PCOS and have been ttc for over two years now.&lt;/div&gt;

      <title>Testicular Failure</title>
      <description>&lt;div&gt;Hello,&lt;br/&gt;&lt;br/&gt;Last month my husband was diagnosed with Testicular failure.  He had some bloodwork drawn and 2 semen analysis tests.  The semen tests resulted in&lt;br/&gt;&lt;br/&gt;no sperm to be found (Aspermazoa?) and the blood tests showed an extremely&lt;br/&gt;&lt;br/&gt;high level of FSH - 38.8  I know that normal is around 7.   Anyways,  the doctor did no further testing, asperations, physical examination even after we asked.  I was wondering if this is normal procedure for a doctor with a patient who has such high levels.  Is it really that cut and dry as for diagnosis of Testicular Failure. We were presented with the option of donor insemination, and are open to it, but we still feel unsettled about whether we had the best care with our specialist.  Should we be asking for a referral? Or do we take a deep breath and try and move forward on a different path?&lt;br/&gt;&lt;br/&gt;Thank you.&lt;br/&gt;&lt;br/&gt;lili&lt;/div&gt;

      <title>What to do after an IVF failure?</title>
      <description>&lt;div&gt;You&#8217;ve now undergone multiple IVF cycles and have yet to take home a baby.  Frustration and cynicism set in and you have feelings of giving up.  Yet, you may not have received a &#8220;full&#8221; work-up which may vary from differing fertility centers.  Some centers may simply continue doing the same thing.  The most likely answer would be to change your medication protocol to try and optimize your IVF stimulation.  Even with this, there is yet much that we can&#8217;t explain as to why some do not get pregnant.&lt;br/&gt;&lt;br/&gt;Some centers may do a blood panel to check coagulation patters, and if there&#8217;s an abnormality, then Heparin and baby aspiring may be prescribed and possibly help with some embryo implantation issues.   Whether beneficial or not, MTHFR mutation is sometimes checked and if abnormal, high dose folic acid (Folgard) is given with or without checking for homocysteine levels by some physicians.  Of more controversy, immunological factors can be measured through serum analysis, such as Natural Killer Cells (NKa) nd if abnormal, then IVIG or Intralipids are given.  IVIG is a blood derived product and is the IV medication routinely used for these patients.  There is primarily a theoretical risk that a blood bourn infection may occur with IVIG. An unproven commodity, intralipids, in my opinion works as well as IVIG without the cost&lt;br/&gt;&lt;br/&gt;Others may perform surgery via laparoscopy to look for endometriosis which may be a culprit for diminishing egg quality and decreased success rates.  There is some evidence and clinicians may agree that burning the endometriosis may temporarily give better pregnancy outcomes.  Sometimes even though looked at before, uterine cavity re-evaluation with a hysteroscopy has at times yielded surprising finding of polyps.&lt;br/&gt;&lt;br/&gt;Male issues are also considered in such scenarios.  Two common clinical sperm assays that are commonly used are the Sperm Chromatin StructureAssay (SCSA) and Sperm DNA Decondensation Test (SDD) which can help determine if sperms is a significant issue and whether or not donor sperm is needed.  L-Carnitine, the active ingredient in many male fertility supplements, is one of the few vitamins that have been noted to particularly help sperm&#8217;s motility.&lt;/div&gt;
