Dear Dr.,

I am going to a fertility specialist right now and he has diagnosed me with PCOS i am currently on CD 11 and have an appt on Monday for an ultrasound and i am unsure of what they will be looking for during the visit? I am on Metformin and Femara and starting to use my OPK today for the next seven days? I just need a little clarification on ultrasounds at this point. Thanks!

I had my first transvaginal ultrasound last week. I am also on Metformin and Clomid. They first measure your cervix and ovaries and then look for follicles and measure them. They look to see how many follicles you have and what size they are--and they look to see if one is more mature than the other (the one that will ovulate). My follies were small--between 10mm-12mm so they do not think I will ovulate this cycle, either. Hopefully the doc will do your u/s--I has a tech do mine and she was less-then informative. Hope this helps!!

THe most important questions you ask are how many follicles you have on each ovary and the size of them. If you have cysts due to PCOS, they will be able to detect them, also. I didn't have any to detect yet my doc says I have PCOS--not sure why...

I really needed that info so now i can actually go there with some sort of an idea about what is going on! How long have you been ttc on those meds? Also, what is considered a good size for the follicles and how many should we have to have a good chance of getting pregnant?

I've been on Clomid for the last 5 cycles and still have not ovulated. I have only been on Metformin for the last month. Apparently the two meds combined has a higher success rate, but I doubt that the doc will let me do too many more cycles of Clomid since there are downsides to taking it. I may have to start injectables soon.

As far as the follicles go, I think that if you get the u/s on day 11 (which is when they usually do them) then I'm pretty sure that they want your follies to be about 15-20mm or 1.5-2.0 cm--but I'm not sure. I had 11 total follies. My biggest one was 10mm and they did not think I would ovulate when my biggest one was so small. I think it is good to have a few follicles on each ovary. Some people only get one or two on each and they still ovulate. More doesn't always mean better.

Hi Amy,

Thanks for the all of the help with this ultrasound. I have taken femara for 3 cycles now but i just recently started the combination of metformin with it. I am on my 4th week of metformin and hoping that the combo this month will work. I am a little bummed out right now because i thought that i would have a +OPK by the time i had the u/s. I am still BDing everyday anyway just in case i get lucky. Have you taken injectables before?

Nope. I've never taken injectables. I am a little scared to try them. There is a high risk of multiples--I think 25% and also I am just a little worried that they, too, will not make me ovulate...that would be frustrating. But, on the bright side, from what I have read, injectables WILL make you ovulate. So we'll see.

A lot of times the docs want you to do the IUI with the injectables. I would rather not do that and just BD, but if it increases our chances, then we may have to. Hubby isn't really into the whole IUI thing, too, but he would do it if we had to.

I have an appointment with my RE on the 28th--next Monday. I'm anxious to see what the next step will be.

I am from Colorado. How about you? It's funny that my husband has the same feeling about IUI too. How many cycles will your RE let you be on clomid ? My dr. just gave me a long list of ways to proceed after we try the femara and metformin together. I am scared of the injectables also!! My cousin had to do them along with IVF and she hated it. I don't do well with needles anyway so i guess your right if that is what we have to do then we don't really get a choice in the matter. I am anxious to hear what your RE will tell you at your appt. and i will fill you in about mine.

Hi amy and crystal, just reading your posts and thought i would put your minds at rest about FHS injections.

yes, to begin with its scary, i mean having to give yourself a needle is daunting.

two cycles ago was my first cycle on fertility meds. my doc did it a bit backwards and gave me FSH before clomid. anyway, the needle is only 1/2 inch long and is so so fine. you will be amazed at how much it DOESNT hurt, it really is mind over matter. i worked myself up alot the firts shot i had and was tense, so i did hurt a bit but the next day it was totally fine! i did in in the tummy, just below and either side of belly button.

i produced 8 mature eggs in right ovary and 5 immature eggs in left ovary. unfortunately with this amount of eggs it was a waisted cycle as i couldnt BD for like 10 days!!!! lol. but i was so excited that it worked!! and next time will make the dose much lower and for less days.

he gave me clomid this cycle and i ovulated on it, so im just happy both treatments work for me. i was on holidays (without my husband) this cycle so missed the 'window of opportunity' but next cycle were gonna give it a real go!!

oh and, they do use FSH injections for ivf because of the amount of eggs it can produce. the specialist asked if i wanted to do IUI and im definatley not ready for that yet, so we just having fun BD.

Thanks for the info. What you mentioned was exactly what one of my concerns was--too many mature follicles. I had 11 last u/s--none of which matured and ovulated. I didn't realize that not all of them will mature on the injections--that's good.
