I am stimming with Gonal-f 75iu for 8 days. My e2 on cd8 was 44 and cd10 was 43....today cd11 I started bleeding like I have my period. My RE cut my dose in half b/c I overstimmed on 150 last cycle. Why is my body not responding to the meds like before? Is there a problem? I had a lap a year ago and have stage III endo. No PCOS as far as I know.

Hi Ashleigh,

The "art" of stimulating someone you're seeing for yourself. You see that on 150 you overstimulated (whereas some would be understimulated), yet when your dose was cut in half, you're definately understimulated given your E2 values and the bleeding that you're having, but wouldn't worry too much about the bleed at this point.

It seems to me that your appropriate dose is going to be somewhere in the middle if not closer to 150IU again...but you can see that I cheated since I have much more info now on you than your RE...but am confident your RE having this new info will give the same recommendation.

If you are on a gonal-F pen, it'll be easier to adjust the dose, but let your RE know before doing anything! The endometriosis, although it may affect egg quality and or block tubes (make sure your tubes are open), shouldn't affect the month to month use of Gonal-F. Well I guess they are just going to keep plugging along at 75. My e2 went up to 57 today and my follicles are continuing to grow. One at 12 and two at 11 and then some 10s,9s,8s,etc.

They said the bleeding should stop soon since my e2 is going up now. My lining of course dropped to 4.9. I hope it starts to thicken. I am still bleeding at this point but hopefully that stops soon.

Next u/s and e2 will be wednesday.

Thank you for your response. I was thinking that they would have to change things. Is it possible that I over-suppressed on the bcp? I was on it for 6wks due to 5 3cm cysts after last cycle. I have had two HSGs and tubes are wide open....I did have a septum too that was removed at the time of my lap.

The bcps should not have oversuppressed you, and is only a phenomenon that we think we see (yet, truly unproven) that there is oversuppression in poor responders, of which you are not one of. The 75IU should eventually kick in, thicken your lining to stop the bleed, and hopefully in a few days, you'll a couple of dominant follicles

Okay well here is the situation. I am still stimming on cd22. Oh Yes. My left ovary has a 17,15,14 and right ovary has a 15. The last three nights meds were 112.5 of gonal then another 112.5 and tonight was 150. I am wondering if egg quality is now an issue with so many drugs in my system? Can they even be any good? I have heard other women say their REs won't let them stim past cd20 as "eggs are no longer good."

My RE's nurse told me they had a girl stim 20 days and she is now pg. Well I am doubting this process. I am suprised he has not cancelled me but he has recognized my low threshold for meds.

I am just tired. Is this a lost cause this cycle?

there is little basis as to stimming longer in good responding IUI patients, and have had several pregnancies stim past 20days. The "poorer egg quality" that you're hearing we do observe in poor responding IVF patients, of which you are not one of them given your low doses of meds...so it truly is not a lost cause and hang in there. Your e2 is on the lower end, but honestly, I (and many REs I know) never check e2 in IUI cycles and provide little extra information...follicle size and growth is most important

Well my cycle failed...no shocker there. we ended up with an e2 of over 400 and the one follie.

Is there a chance that I was oversuppressed from BCP? I was on it for 6weeks prior to this cycle.

We have decided to move on to IVF b/c ins covers it and our chances are greater for success. I am nervous b/c bcp is part of the protocol and if I am going to be oversuppressed again I want to address it b4 hand.

We're sorry Ashleigh that it didn't work, and IVF does provide the best possible chances for success. Oversuppression is a phenomenon that we don't quite understand all to well, but again, primarily happens to poor responders, of which you seem to be a "good responder". BCPs will most likely be Ok, some of us REs believe it or not worry more about the use of lupron in poor responders, which makes some sense when considering the possible effects lupron can have on the ovary. We're hopeful that the IVF will yield you success and like always...

Hi Jonah,

One should first see a Urologist specialising in male fertility, since there may be a problem that can be treated. Once this is established, then there are a few products such as FertilityBlend, FertileOne, Proxeed, etc that can help. Otherwise, if the sperm count doesn't come up significantly, then IVF + ICSI may be required for conception
