stopping Metformin and Progesterone at 12 weeks

Hello Dr. Q,

Is it safe to stop Metformin and Progesterone suppositories at 12 weeks??

I have PCOS so will my insulin levels be off the chart and my sugar level increase? and will my progesterone level decrease if i stop all these medications at once?

I understand many continue taking Metformin through the first trimester, and good data from the PPCOS study seems to point that there is no benefit in doing this to prevent miscarriages, I personally don't do this, ok to stop both but talk to your doc about this...

The fetal placenta by 10 weeks, espcecially 12 weeks produces more than enough Progesterone to where supplementation is usually not necessary...

My OB has asked me to stop the medications. But she said i should try to have low carb diet. Do you have any information or website where i can look for the low carb diet for pregnant PCOS women?

is it true that i am at a greater risk of having gestational diabetes? what are the risks to my baby?

If you have PCOS, then you may have insulin resistance which only increases in pregnancy, increasing your chance of developing gestational diabetes...

I would not recommend a low carb diet (atkins) say as this can be notorious for creating ketones, which may not be a big deal if not pregnant...However, ketones can severely impair brain function in the developing fetus...so regardless of what someone out there may put on the net...not sooo much of a good idea.

The general recommendations for eating in pregnancy in terms of breakdown are:

60% Carbohydrates

20% Proteins

20% Fats (divided equally into Saturated and NonSaturated)

Normal Caloric Intake:

Normal BMI: 35Cal/kg/day (approximate 2200 Cal/day)

Overweight BMI: 25/kg/day

Hope this helps, and there's some wiggle room in this but wouldn't deviate too much...
