Question about today's doc visit

Hi Dr. Q:

I went into my fertility center today in order to begin my third IUI cycle with Clomid (did 2 natural IUIs and both failed, due to low sperm count: 2.3M, 61% motility). I'm nervous about taking the Clomid (50 mg) since it's my first time taking any fertility meds -- or really anything other than antibiotics! I'm hoping you can help mitigate my concerns.

Had a sonogram today and they counted 17 follicles (9 on right ovary, 8 on left). The last time I had an antral count of 10. Nurse called my ovaries "active."

Since I have so many follicles, do you think I'm at increased risk of OHSS or other Clomid side effects/complications? I'm really worried that the Clomid will make me feel ill.

Also, is there really any point in trying a medicated IUI when sperm count it this low?

Christine...I know I am not Dr. Q but I thought I share with you my experience. My DH also has low sperm count (4million prewash) and I have PCOS (I have 20 cysts on my Rt ovary and 25 on my left). I started wiht 50mg of clomid the 1st cycle. I was nervous in the beginning but kind of excited at the same time. 50mg of clomid definitely jump started my system. I ovulated but missed the egg because I ovulated on CD 10 which was way early. Then I tried again 2nd cycle on the same dosage, it did not work! I had 6 follicles >10mm and they all regressed to 4mm in 3 or 4 days. so my RE increased my dosage to 150 which is a good jump from where I was.

I had my appt this morning to get my final measurement of the follicles. I have 2 dominating follicles (one on right = 19.5mm and one on the left = 20.5mm). I have been taking baby asprins and it thickened my lining by 3mm in 2 days! Yay! So I am going to take ovidrel tonight around 11PM and go in on Sunday for my IUI.

As for side effects of the Clomid...its tolerable. It is not too horrible, at least for me. I had some mood swings, hot flashes, bloating, headaches....but I had those symptoms from PMS too so not a huge deal. Trust me, I was scared in the beginning but you know what? Its all for a good cause!

Clomid will help you to get a good mature egg and IUI will help with the low sperm count.

they are just telling you how many follies they see there but that doesnt mean that you are gonna make that many mature ones :)

Just like if you didnt take anything you would still be able to see all those follies in your ovaries.

I didnt have many side effects really, just some minor bloating and then hot flashes, more at night tho. I took the pill at night when going to bed and i truly believe that is the reason i didnt have a lot of the side effects that ladies talk about. I will tell you that ovulation was not comfy at all - i wasnt monitored so i have no idea how many follies but ovulation was actually painful for about 6 hours or so.

Thanks so much, Angie and Chels. I really appreciate you sharing your experiences with Clomid. I'm an anxious person, generally, but especially about medical stuff. I think that once I take a pill and feel fine, I'll feel better about the whole thing. I hope Dr. Q responds, too, as I'd really like to hear his take on my situation. :)

Update: I just got a call from my nurse that my health insurance provider hasn't yet approved my IUI w/Clomid for this month! This means that if we proceed, and for some reaon they don't approve it, DH and I would have to pay for this cycle 100% out of pocket, versus the 50% with the insurance coverage. Needless to say, we're NOT doing that! :)

But honestly, this news is a blessing in disguise. For the past 2 days, I've been obsessing about taking the Clomid. I know it seems silly to many of you, since we're all so focused on achieving our goals: to hold a precious baby in our arms. Unfortunately for me, the prospect of taking this medicine sparked my anxiety issues and made me feel really uncomfortable and on edge -- even physically ill. It's difficult to understand if you're not familiar with anxiety disorders, I know, but please take my word for it: It's not pleasant. Although my anxiety is mild (yes, I see a therapist about it), it still finds me and it's sometimes debilitating. So, we're going to make this cycle another natural IUI, while we wait for approval and while I hopefully have more time to get comfortable with taking fertility meds.

I want to thank everyone for giving me really great info and just being supportive. I'll still be on the boards, but won't annoy you with questions about whether Clomid will kill me. :)
