prometrium vaginal inserts??

Hi Dr. A friend is using prometrium inserts 2x a day. A nurse mentioned something about poking a hole in the insert. When she does this it kinda breaks apart and gets messy. What is your recommendation on using the inserts? THANKS!!

Poking a hole isn't necessary, just needs to be placed with the applicator
