Primary Ovarian failure? Help please

Doc,

I am currently on 300iu of gonal-f going up to 375iu. I started with 75iu and then to 150 then 225 and all i got was a 51 on estrogen then 81 and now 90 after 13 days of injections. Does this sound like I could have Primary ovarian failure if 300iu only took me to a 90? I have PCOS and have been ttc for over two years now.

Not necessarily devine, could be that your ovaries are resistant to the medications, and I have seen this many times with PCOS patients where large doses eventually have to be given for ovulation to occur...

Especially if your FSH is normal, primary ovarian failure is not the cause, and PCOS is most likely what we're dealing with

What is the largest dose that is safe? Thank you for responding today has been tough.

A cealing effect is noted in which no matter how high the dose, the response will not be any greater after 450-600IU of gonadotropins

okay, well what is the next step if gonadotropins don't work?

they will work if your FSH is normal...the problem maybe that you'll make too many follicles for an IUI and controlling the number of follicles may be the issue (the risk for multiple births is too high)...if it can't be done, then most go on to IVF

Okay. I have an appt on the 30th to discuss options and this is my first round of gonal-f. It just so depressing when things don't work the way you want them to. I feel lots of little twinges in my overies when we went to 375 iu today could that be a good sign? Sorry for so many questions. :) I dont know what I would do w/o you guys here :)
