I am 21 and my husband and I have been trying for about a year to have another baby. I don't have regular period and started going to a new doctor. After my apt., she put me on provera and after taking it for 10 days I had a period, then I started metformim to cause me to ovulate, well that didn't work. She now wants me to go back onto provera but I don't understand why because then my next step would to go back to the metformim, but it didn't work. I still haven't had period since going off the provera the first time. What would my next step be or are there any other options?

Hi Christina,

Sounds like you have PCOS and a combination of Clomid and Metformin may help you after you finish the Provera. Talk to your doctor about this to help you ovulate, and you can research more on this topic on this site to learn pregnancy rates, etc. If this combination doesn't help, then you may need injectable gonadotropins given by an RE to help you start a family

But my question I guess is more what do I do that I have already tried the provera and then done the metformin, I have been on metformin 1000 mg a day for the past 36 days and still haven't ovulated or had a period, so do I need to talk to my doctor about starting something different??

Hi Christina,

Talking with your doctor is a great idea so that Clomid can be added to your regimen of metformin so that you can ovulate, and hopefully get pregnant via this method.

Here is a new one for ya... I went to pick up my prescription today to go ahead and start the next set of provera, when I went to pick it up the doc double the doses, instead of taking 10mg a day now I am taking 20 mg a day, please explain that one!

Hi Christina,

The dose that I've usually heard is from 5-10mg/day of Provera for a withdrawal bleed. Otherwise, you can double check with your doc about the dose.

Well I just got off the phone with my doctor, the provera doses were wrong so I am only on 10 mg a day. She then decided that the next step would be to continue taking metformim and add Avandia one a day, if that didn't cause me to ovulate then to come back in, was this a good move or a bad one??

Hi Christina,

Sounds good, talk to your doc about the new theoretical risks of Avandia/Rosiglitazone (have talked about this on this site and you can search for this), but also ask your doc about being on Clomid

We dicussed the risks of the Avandia and she said because I am young and healthy that it shouldn't cause any problems, do I still need to try Clomid or see if this helps first and if not when I go back in recommend it?

Okay new question...I started taking the Avandia today and my blood sugar seems to be dropping when I don't eat every 2 hours or so...I noticed that hypoglemia is one of the side effects...should I continue taking it or stop taking it until I can call my doctor on Monday?

Thank you very much I am concerned b/c in high school I had hypoglemia and I don't want to have to stress so much about a special diet, if these pills aren't going to help would be the next step or pill to start... also I forgot to mention earlier that the metformin is also starting to make me sick...??

Metformin does have these effects that make you feel "sick", if anything, see what happens once you stop the Avandia, and you can talk to your doc about dropping the dose of metformin also until your body gets used to it...

I have been on the metformin for going on 2 months and it started doing this at the beginning but seems to be getting worse instead of better...it wasn't as bad when I was only take 500 mg for the first week but since then I have been taking 1000 mg a day should I drop it back down to 500 mg b/c she said I could do this if the side affects got to bad...?

Also, if she takes me off of the Avandia what should I start next, would it be Clomid? or something different?

Hi Cristina,

I was wondering, have you specifically been tested for PCOS? There are tests to corroborate this is a problem; I think they check the LH to FSH ratio. I wanted to make sure your doctor has checked (as far as possible) to find out what the problem is for you.

Not a bad idea to make sure PCOS is the culprit here, and if so... you will probably be on Clomid to help you ovulate while on Metformin. It's not uncommon that when metformin is increased from one pill to the eventual 3 pills a day, that you start having greater side-effects. Talk to your doc, try to be on 1500mg/day to get the maximal effect. Now that you're off the Avandia, check to see if the dose you're on for a few days continues to provoke side effects. My doctor hasn't checked for anything, I went in and told her the problems I was having and she started me on provera immediately, then I started the metformim and so onand so for, but she hasn't done any tests to see exactly what is wrong or going on...

According to that post and other things I have read I don't have any of the sypmtoms other than irregular period, so if PCOS isn't the cause what else might be the reason we are having trouble conceiving?

I also had an ultrasound done about 6 to 8 months ago and everything came back normal, so that also leads me to believe that it isn't PCOS, my ob/gyn wanted this information and hasn't request anything new be done...

Hi Christina,

It makes me wonder since metformin and/or Avadia are only given to PCOS patients when trying to conceive, and not to any other particular infertility patient. Elevated levels of testosterone are not necessarily noticed, and don't always give increased hair growth, especially in certain ethnicities. Most people who do ultrasounds, unfortunately also don't report "PCOS looking ovaries" and simply state normal. Given this info, talking to your doctor would make the most sense to discuss the use of these medications on you and/or your diagnosis.
