New Semen Analysis Question

I have had a second semen analysis and have the following information:
27 million sperm per millimeter
60% not moving
10% move, but no go
10% move very slow
20% moving pretty good
Kruger test says 2%
The nurse told me that they like to see 4% for the Kruger test.
They are now going to do a hormone test on me.
My questions are:
1. What can be done with all of this to get my wife pregnant?
2. What are the procedures that are available and might work?
3. I had undescended testicles as a young boy and had that fixed at about the age of 4 or 5… are my results in line with that?
4. Could I have a hormone problem that could be causing the results that I have?
