Concerned about no monitoring on clomid

Thank you for taking the time to read this. I'm 31 and have been trying for baby #1 since Jan. 2006. My husband and I started seeing an RE in April of 07 and all tests came back normal for both of us (SA, HSG, FSH, progesterone). I am hypothyroid, but all my tests come back normal so I am on the right dosage of synthroid. I have never been pregnant, and my husband has never fathered a child.

I am in my third cycle of clomid. My RE gave me a prescription for 6 cycles of clomid and said to come back if it didn't work. I was a tad surprised that there would be no appointments in the middle, no monitoring of any kind. I have ovulated every month on my own prior to this, with cycles ranging from 33-42 days.

I find I am feeling side effects of clomid more each cycle. For the past few weeks I have had minor pressure/cramps continually - much like PMS cramps, but milder. Should I be concerned that there is no monitoring? I have read about the affect clomid has on cervical mucous - I am concerned that this may be happening to me, but I won't know until my 6 cycles are up.

My RE is very busy and is the only one within 8 hours driving, so switching isn't an option unless I'm willing to fly somewhere.

Any advice?

I forgot to add that my luteal phase is 9-11 days. Once it was 12, but usually 9 or 10.

Hi runjc,

This is a tough one, because even monitoring you would be very difficult since you're so far away from an RE. It seems to me you have unexplained infertility, and have written about success rates in this site...I don't know if I blame your RE for not wanting to monitor you if the pregnancy rates are not the greatest on Clomid and you have to drive so far just to visit the RE for monitoring. People do get pregnant taking it every month, and if you're concerned (cervical mucus abnormalities don't happen to everyone), you can always switch to letrozole. You may also need luteal phase supplementation (vaginal progesterone) since this seems to be short, and would contact your RE.

Since you live pretty far from one, and if the Clomid and/or injectable success rates aren't acceptable to you, you may want to consider doing an IVF cycle with your RE to make your trips worth while, and your RE will open up much more to your treatment if you express interest in this.

Dr. Q, thank you so much for your response. I'm sorry I wasn't very clear about my RE.

She is local, only about 10 minutes away. But if I wanted to switch (since I was concerned about no monitoring during the 6 clomid cycles) then switching isn't an option because the next closest RE is 8 hours away.

Thank you for the info. on letrozole, and progesterone. I will be sure to ask her about this next appointment.
