Chlomid or Follistim

Hi dr.Q.

Unfortunately my PC seems to be playing games, or I don't know the reason my couple of last questions haven't been answered. Hope I don't bug you too much, lol.

Well as I wrote I started my first round of Chlomid this month. My doc pot me on the smallest dose--50.I asked you in my previous question about one very silly and maybe crucial mistake I have done. Basically, I confused my period starting date and started Chlomid on day 2 of my full flow period (day one I had a flow but kinda brownish). So, the question is how serious this mistake can be? Will it bring any serios problem? Basically I was supposed to take it days 3-7 (and it turned out 2-6). On the first day I felt very bad hot flashes, and then I was just pretty nervous and had mood-swings as a crazy girl. Now I am on day 9 of my cycle and experience just some moody things. On Saturday (day 11 of the cycle) I have an app at the doc's for estrogen blood work and ultrasound. In the psitive scenario what should it be that day? And by that time should they be able to tell if Chlomid worked for me?

One more thing-- will it be better if they recommend using OPK or daily ultrasounds for catching the right moment for IUI? We were told that IUI would be made twice with 24 hours difference, that is supposively increases chanced of conceiving. Is it so?

And as I tagged my question, what kind of medication is Follistim? What is the main difference between Chlomid and Follistim? How much more expensive is it? And why don't docs use that one first if it is a better medicine?

Sorry for so many questions. But I am pretty excited about everything these days. Please, answer my questions and wish us luck with IUI.

Don't worry too much, a day 2 start will be perfectly ok and these flushes and mood swings are very common side effects from Clomid. By now, you should be able to tell if the clomid is working as you should hopefully be able to see a follicle growing. An ultrasound may help, but an OPK is just as good when solely using Clomid. As far as two IUIs, one is just as good as two, and personally, I only do one.

Follistim is an injectable gonadotropin that directly stimulates the ovary by it's FSH action to create one or more follicles. It is definately a much better medicine with higher pregnancy rates, but you would need an RE to monitor you with this. I'll wish you the best, and good luck with your IUI!

Thanks a lot for your answer DrQ. I feel better now knowing that I took Chlomid all right. As for its effect, I can tell by now that on day 7 of my cycle I had lots of egg-white mucus, then yesterday it was less but my cervix position was very high( I could hardly reach it). Today I have some egg-white again. Honestly, so much egg-white mucus I haven't had for about 2 years...So, I guess Chlomid is working.

As I wrote, my ultrasound and blood work is on Saturday That will be day 11 of my cycle. If previously my ovulation was on days 18-21, what might be the time this month with Chlomid? Will ultrasound be able to identify at what stage I am? And could it be with all that mucus, that by the time of ultrasound I already miss ovulation? (though I pray God not to let it).

Thanks for your good luck wishes regarding IUI. I will keep you informed what will go on and I guess I might write some questions even before we see the result.

Thanks a lot for your help, and I do love this site. God Bless you for this work!

Hi Natasha,

The ultrasound you're going to have will tell you exactly how far along you are, and given your ewcm, I'm confident you're developing a follicle. If you're truly worried that CD 11 may be too late, buy an OPK and start checking now, that way you will absolutely know you won't miss ovulation...Good Luck!

Hello.

A little strange thing is happening to me-- on day 7 of this cycle (with Chlomid 50 on days 3-7)I had a lot of egg-white mucus. But today -- day 11-- I can hardly find that nice stretchy stuff... Does it mean that my folicle stop growing? Or why can it be so?

Tomorrow is doc's appt, but I just decided to share this worry with you and ask your oppinion.

Thanks.

Natasha,

It's hard to say what's going on given your change in mucus and we're hoping your follicle is still growing fine, but please let us know tomorrow after your ultrasound as this would hopefully let us know what's going on.

Hello DrQ.

Today I was at the doc's. So, estrogen level is 89 point something (that they said is ok), and by ultrasound they found 2 follicles growing sizes 16 and 17.They said that my body reacted very good for Chlomid (3 knocks on the wood), and they want me to start using OPK, and also to come to them on Monday again for estrogen blood work and ultrasound again.

So, from your point of view-- on cycle day 12 do I have good things developing?

Please answer as I will be waiting for your reply impatiently.

God Bless You and the best of luck to everyone here.

That's awesome Natasha!!! Two follicles of that size should trigger a positive OPK in about 2 days. We're sprinkling some more babydust around here when you get your IUI done early next week, and keep us posted

Thank you so very much for support!

I did one OPK yesterday (day 12 of the cycle) just to be on the safe side not to miss the surge. It turned out negative. Today I have been experiensing some dull disturbance downthere for the most part of the afternoon, and around 5 pm it was pretty annoying pain, but OPK done at 6pm showed negative. I am going to use another OPK tonight around 9, but I guess I do feel those tiny eggies developing (also I found today LOTS of whitish coloured pretty stretchy mucus. I just pray God to hold my ovulation till at least tomorrow evening since I don't want to miss IUI that should be done at the appropriate time. Tomorrow we are having another ultrasound and estrogen blood work. What do you think,Dr Q, if they see that follicles are big enough, are they still going to ask me to use OPK or they will want to start IUI? (oh, by the way, Saturday's estrogen level was 87.3-- I heard results one more and now make correction to my previous info.I tries to find info in the i-net about normal level of estrogen just before ovulation of a woman 28, but I couldn't find. Could you give me at least an idea if my level is considered to be all right?)

Well, I will keep you informed about all our things, and please, pray with us.

Sinserely,

Natasha.

Natasha,

Most RE's would recommend you trigger ovulation with an hCG shot, and am sure some of our users have undergone this. With an hCG shot, they'll tell you exactly when to go in for the IUI. Otherwise, they may just wait for your body to give you a positive OPK and then do the IUI the following day, but they won't do an IUI without any one of these two things happening. Most women with one follicle at ovulation should have an estrogen level of 200-250 pg/Ml. We're all keeping our fingers crossed for you and keep us posted.

Hello DrQ.

Today my progesterone level was 124 and they told that one follicle is very nice and ready. Thus, they want us to come for IUI tomorrow at 7.30am and then the next day the same time (our doc always does 2 IUIs).They didn't mention those trigger shots. Honestly, he did tell us that the shots would be only in case 3 rounds of Chlomid didn't work. So, no shots tonight (for the better or worse I am not sure).

What surprised me is that they didn't mention the size of the follicle today. Last time they told about 2--16 and 17. The nurse at the ultrasound was happy with my right side follicle, but when I asked her about left side, she said that there was some activity there but she wasn't sure if it was still going good enough. I am puzzled a bit-- what could have happened to the second follicle if they were almost the same sizes? Is it that just one prevailed and the second went "to sleep" again? How does it work, DR Q? And if the 2 follicle doesn't mature enough, what happens to it-- does it die or next cycle it tries again? Sorry for such questions, but I have no idea about this and it's very interesting to learn.

So, tonight I want to use OPK (mostly for myself out of curiosity), and the next 2 days are going to be pretty important for us. Please, keep fingers crossed.

Oh, one more question-- at my work sometimes I have to carry up to 33lb few bags. Will it be better to avoid doing this after IUI or it is still ok and won't hurt?

Ok, that's all for now and thanks in advance.
