I have undergone some medical tests for which following is the report

Lutenizing Hormone - 9.85 mIU/mL

Follicle Stimulating Hormone - 5.63 mIU/mL

Prolactin - 9.55ng/mL

Estradiol - 37.05pg/mL

TSH 3rd Generation - 2.18 uiU/mL ( u - micro )

Progesterone tested on 21st Day - 18.99 ng/mL

I have Sedimentation Rate of blood as 36 (0-20) MM/HR which is not in range ( 0-20)

Glucose Fasting - 93 mg / dL

Glucose Pranadial - 107 mg/dL

Insulin 32.10 mU/L

Testosterone, Total - 58.60ng/dl

Testosterone, Free - 1.4pg/mL

% Free Testosterone 0.24

DHEA-Sulphate 67.1 ug/DL ( u - micro )

Kindly explain what my report says and what are my chances of conceiving.

Hi Kusum,

Everything appears to be normal, except that your insuin post prandial may be alittle higher than expected...ask your doctor and you may benefit from either Metformin or Avandia. It's looking good, I'm assuming the tubes are open and the semen analysis is normal. If so, you may haven unexplained infertility, and have written about pregnancy rates using different modalities on this site that you can search for. Review the percentages and talk to your doc about which treatment is best for you. Good luck, and please keep us posted

Thanks Dr. Q, for your prompt reply. I really thank the person who has told me about this website. I was so eager to ask questions from doctor. But, not all my queries were answered. I really appreciate your effort of comforting others by responding to there queries. Doctor my best wishes are with you.I am really impressed!!!!!!!!!!

Dr. I have missed something in my previous note.

I was detected with pcos.

I have undergone ultrasound( From 9 th Day follicular mantasy was done) and i was ovulating on 15th day and my cycle is of 28-30 days.

Also, I have undergone Endometrial Biopsy ( HPE and Mycobacterium ) on 1st day of periods for which my reports were normal.

But, for the past few(3-4) months, I was having very scanty periods, just a day or 2 that also very light.

My Husband's report says count is 12million/ML.

viability - 10%

motility - 10

Red Cells - 1-2/HPF

Pus Cells - 5-7/HPF

Bacteria - Detected

My husband has undergone colour doppler which came out to be normal.

My Husband has been told to have

1)Andro Gaurdian twice a day for 3 months.

2)Confido also twice a day for 3 months.

For me Gynaec has told the following:

1)ESR is high

2)Insulin is high

3)FSH/LH ratio is imbalanced.

She has put me on following medicines for 3 months:

1) Duphastan 10mg -2 times from day 16 - day 25

2) Lynoral 0.05mg - 1 time from D5 - D21

3) Bigomet 500mg - 1 time

4) Gaurdian -1 time

After having these medicines for 1 month I was getting severe cramps and pain in my legs and way not able to carry out normal routine. So, I contacted my gynaec she told me to stop Duphastan and Lynoral immediately and told me to have calcium for 1 month.

I am 27yrs old and my husband is 33yrs. We don't have issue.

I am very much confused,I have following queries:

1) What are the chances of our conceiving ?

2) Can my husband's semen count be increased with

the medicines he is having ? Is he having right

medicine ?

3) Can I say that I have diabetes/sugar problem as my

insulin is raised.What are the precautions I need

to take that my insulin level does not rises.Is it

due to pcos.

4) Also, as my ESR is raised does it means that i have

thick blood. Tat's why medicine like Duphastan and

lynoral are not suiting me. What can I do to reduce

my ESR ??

5) My Gynaec has told that I have been given duphastan and lynoral to induce regular periods.And it worked last month as i was having this medicine. I have regulated periods. But, as of now I am not having these medicines then I will not have regulated periods?

Will it effect my chances of conceiving.

6) Doctor one major thing, my stamina is reduced very much . Especially my limbs are effected the most. Whatever, medicine I take, I start getting cramps and pain in my legs and sometime sweling also. What can I do regarding this ??

Hi There,

These medications are not available in the US, and can only give you my best opinion given the circumstances.

It appears that your husband's sperm motility is too low for a natural conception to occur, and would have this repeated. If it remains low like this, then your only good chance of achieving a pregnancy would be through IVF + ICSI. There are some products available here in the US that may help sperm, such as FertileOne, FertilityBlend, etc. Not so familiar with Confido or Gaurdian.

The medications you're receiving will give you periods, but chances are you may not be ovulating because you're on these hormones...in a sense, it's like being on the birth control pill that gives you periods but it's purpose is to keep you from getting pregnant, guessing because you're using the same hormonal pills with possibly these same effect. I'm thinking your PCOS is really keeping you from having periods and your doc is more concerned in you having periods for your safety than for you to conceive (just a thought)

Given all of this information, I would definately see an RE in your country, if not, contact one here in the US who can better manage your situation. In terms of your stamina and limbs, would consult a general practitioner for more information.

Hi Kusum,

The diagnosis for diabetes is primarily done when your fasting blood sugar level is >126, so that I don't think you have diabetes but you maybe insulin resistant, given that your insulin is raised. This raised insulin is most likely due to a little excess fatty tissue, and the excess insulin triggers these PCOS symptoms. If you lose weight (if this is the case), then you may return to normal.

You don't have to restrict much from your diet, just eat smaller portions throughout the day and exercise, otherwise, consult a nutritionist who can better regulate your weight.

You were on birthcontrol pills before (duphastan..) which will keep you from getting pregnant and not allow you to ovulate while on them. We will know more once you're off of these. In terms of the stamina and limbs, you may want to consult a general doctor for this and the management of your ESR...
