Hi Dr. Q,

My RE recommends that we try IUI first, then move on to IVF/ICSI, because of my husband's sperm values (low count=18M, 25% motility, low morph.). I've done a number of tests, and he told me there is no evidence of ovarian aging at this time, which is good.
New comment from Dr. Q

Yes, the 20% chance is the overall rate

However, when one has difficulty getting pregnant over a year if <35, then the chances are far less than 20% in this subselect group of patients

A 10% chance is a significant boost over doing nothing, and Clomid or Letrozole should be considered to give this value, as a Natural cycle/IUI that you're proposing in doing, even with a mild sperm factor

Why are chances for IUI so low?

Hi Dr. Q,

My RE recommends that we try IUI first, then move on to IVF/ICSI, because of my husband's sperm values (low count=18M, 25% motility, low morph.). I've done a number of tests, and he told me there is no evidence of ovarian aging at this time, which is good. We're first going to try natural IUI, because we first want to see how my husband's sperm reacts to washing. If necessary, we'll IUI with meds the next time.

I found it distressing that despite me being OK, and the fact that the IUI puts sperm much closer to my eggs, he gave us about a 10% chance of getting pregnant. Isn't there a 20% chance of conceiving naturally each month? I don't understand, since I thought IUI would give us a boost of sorts.

Can you shed any light on this? Also, do you think IUI can work for male factor like that, or are we crazy?

THANKS!
