Dr. Q, I am on 500 mg Metformin twice daily. My doctor said to wait 2 months to see if my cycle returns to normal with the Metformin alone. Is this too long? Everything I read says that the Metformin alone is unlikely to get my cycle back to normal and help me conceive. Should I be telling my doctor I want to do more or finding a new doctor who is more aggressive in their treatments? Thank you!

Dr. Q. Are there other forms of PCOS other than insulin resistant PCOS? I have started fertility treatments and have been given information on PCOS,and was told I have an increased risk for Type II diabetes due to PCOS but this is the first time I have read about the insulin resistance deter ovulation. Wondering if I should mention Metformin to my OB. I did not realize Metformin was also used for this purpose.

my doctor said I probably have PCOS but he did not do any test b/c at the time I wasn't trying to get pregnant he just put me on BC pills. The BC pills really helped with alot of the symptoms but now that I am of bc pills I notice some of the symptoms returning. I go to my Dr next tuesday to tell him I want to get pregnant what do you think he will give me?

Hi Dada if you have a question for dr. q it is best to post a new question many times it does get missed with prior threads:)))).

However with all meds there is side effects and such and only you and your dr can look more into what level is right for you. Also all meds do have a list of side effects either on the insert that comes with the meds and/or the net is a great source. Plus per this site there is a list of all side effects and you can use the search box for clomid side effects and pre-seed. Also pre-seed.com will advise you of what it does.

keep us posted on your ttc journey and ****babydust***

No AF, but I have AF cramps, so im sure she will probably come. Still have lower backache and what not. Needless to say I didnt test this morning, because of the temp drop.

If AF comes hopefully better luck the next cycle, but I guess only time will tell.

Hope the doctor goes good!

Lots of love sweetie....BABYDUST!!!

What I do is set my phone alarm for like 7:40 every morning because I get up around 7:45 every weekday to shower and then leave for work. So I just have it set for 7:40 every morning even on the weekends, and then I go back to sleep on the weekends.

It takes like 4 minutes at the most, and then just write it down, and go back to sleep, and get on the computer later, pretty simple.

Yep I have used OPKs as well. I got a positive and then I ovulated and knew that from the OPK and from my temps.

We have been married since August 12th, 2006, so a year and a couple months. We have been together and lived together for 5 years 3 months total. We just waited a little bit to get married.

Thanks for all the dust sweetie, and lots and lots of OVULATION DUST, BABYDUST, BABYGLUE, and POSITIVE HPT DUST too!!!!!!!!

Lots of love!!!!!!

(((((HUGS)))))

Polycystic Ovarian Syndrome (PCOS)

PCOS is a common condition affecting about 15% of reproductive age women and can usually be diagnosed by your medical provider if you have 2 of the following 3 present: increased bodily hair growth or increased blood testosterone levels, absent or infrequent periods, and/or the appearance of polycystic appearing ovaries on ultrasound. Women with PCOS have a higher risk than normal to develop diabetes, heart disease, and uterine cancer. Furthermore, overweight PCOS women who get pregnant run the risk of developing gestational diabetes, fetal demise, and many other pregnancy related complications. It is therefore recommended that all overweight women with PCOS undergo weight loss prior to planning their pregnancy. For some of these PCOS women who wish to undertake these pregnancy risks, there are several ways to help them get pregnant.

There are several oral medications that can be given to promote ovulation in women with PCOS. Most experts agree that an excess amount of bodily insulin can deter ovulation; as such, diabetic drugs such as Metformin or Rosiglitazone are commonly used to help increase the probability of ovulation in insulin resistant PCOS patients. Another class of medications, ovulatory induction agents such as Clomid and Letrozole, can be used to help PCOS patients. Most studies that have evaluated the combined use of an insulin drug such as Metformin with an ovulatory agent like Clomid have repeatedly shown a higher pregnancy rate than when individual drugs are used alone.

The majority of people who get pregnant using Clomid do so within the first 3 cycles, with no further benefit noted after 6 failed cycles. If a pregnancy isn’t attained with Clomid, then injectable medications with human Menopausal Gonadotropins (hMG) or Follicle Stimulating Hormone (FSH) should be given. However, risks of multiple gestations and ovarian hyperstimulation (OHSS) should be considered since multiple eggs can be created following a treatment plan. To help avoid these complications, patients require proper ultrasonographic monitoring from their medical provider. Intrauterine Inseminations (IUI) should be considered to be performed in conjunction with the oral and/or injectable medications listed above to help increase pregnancy rates.

In Vitro Fertilization (IVF) provides the highest pregnancy rates out of all available procedures. PCOS patients, however, have slightly decreased pregnancy rates because their egg quality tends to be poorer and they encounter higher rates of OHSS. Obese patients with or without PCOS have also been shown to have diminished pregnancy rates, contributing to this overall finding. If possible, it is always recommended that overweight patients lose weight not only to increase their probability of pregnancy, but to also live a healthier, longer life.
