I got pregnant again in April and the same thing happened. I am still going to through the testing, but there is really nothing you can do to prevent it unless there is a proven problem( Hormones, sperm, etc.) I am now seeing an RE, so when I know I will post something. Its due to chromosomal problems with the embryo.
